    CERTIFIED REGISTERED NURSE PRACTITIONER 

    The primary function of the Certified Registered Nurse Practitioner is to assure the provision of quality primary care services to Chester County families.  Services of the Family Nurse Practitioner at LCH include the following: physical examinations and immunizations, chronic disease management, acute episodic care for infants, children, adolescents, adults and geriatric clients.

    The primary function of the Medical Assistant is to assist the clinician in the provision of primary healthcare services to the client and to act as an advocate for the client.  The Medical Assistant collects health information, performs phlebotomy and vital signs, assists with procedures as required, and records medical data in the electronic health record.  Medical Assistants as a team are responsible for patient flow, and health center duties including stocking rooms, maintaining inventory, medical supplies and medical equipment, and performing quality assurance.

  • MEMBER RELATIONS SPECIALIST

    MEMBER RELATIONS SPECIALIST

    The MRS provides vital customer service via telephone and in person. This position is responsible for answering calls on LCH’s main line, routing calls to appropriate staff, making appointments for the health center and providing basic information. When functioning in the role of front office reception, the MRS directs members to the proper service, performs client intake and assessment, provides check-out services, collects and records fees, and provides client services (appointments and inquiries).

    Teachers instruct adult learners in one or more of following areas: English as a Second Language (ESL), GED, Computers and Citizenship. They are responsible for understanding the goals and educational needs of their individual students, encouraging and supporting them to facilitate learning, and providing high-quality, stimulating classes that result in student retention and progress.

    The Bookkeeper is responsible for the accuracy, timeliness and efficiency of the day–to–day bookkeeping at LCH including payroll, banking, petty cash, accounts receivable, accounts payable, vendor file maintenance, revenue recording, payroll and employee records management, vendor records management and patient payment recording. This individual will adhere to all governmental accounting standards.

  • CASEWORKER

    CASEWORKER

    The Caseworker at LCH assists clients by providing quality services in the most efficient and effective manner possible; collaborating with clients to assess needs, develop goals, implement plans, and obtain appropriate services. Additionally, casework encompasses close collaboration and advocacy with government and private agencies, other professionals and members of the community. Casework is an important component of services offered at LCH and is based on the recognition that a trusting and empowering relationship between the Caseworker and the client is central to restoring and enhancing the client’s independent functioning.
