    CASEWORKER

    The Caseworker at LCH assists clients by providing quality services in the most efficient and effective manner possible; collaborating with clients to assess needs, develop goals, implement plans, and obtain appropriate services. Additionally, casework encompasses close collaboration and advocacy with government and private agencies, other professionals and members of the community. Casework is an important component of services offered at LCH and is based on the recognition that a trusting and empowering relationship between the Caseworker and the client is central to restoring and enhancing the client’s independent functioning.

    CRNP

    The primary function of the Certified Registered Nurse Practitioner is to assure the provision of quality primary care services to Chester County families.  Services of the Family Nurse Practitioner at LCH include the following: physical examinations and immunizations, chronic disease management, acute episodic care for infants, children, adolescents, adults and geriatric clients.

  • MEMBER RELATIONS SPECIALIST

    MEMBER RELATIONS SPECIALIST

    The MRS provides vital customer service via telephone and in person. This position is responsible for answering calls on LCH’s main line, routing calls to appropriate staff, making appointments for the health center and providing basic information. When functioning in the role of front office reception, the MRS directs members to the proper service, performs client intake and assessment, provides check-out services, collects and records fees, and provides client services (appointments and inquiries).

    MENTAL HEALTH THERAPIST

    The Mental Health Therapist will provide psychotherapy for adults, adolescents, and children using individual, group, couple, and family modalities. LCH provides counseling and intervention services to help clients reach their full human potential. The Mental Health Therapist will collaborate with members of the health care team, the community service team, and the client/family to remove barriers to health and wellness.

    PSYCHIATRIST (Part-Time)

    The part-time Psychiatrist will address the behavioral health needs of clients by conducting psychiatric evaluation of clients, prescribing, and managing psychotropic medications.  The Psychiatrist will coordinate with other behavioral health and primary medical providers and support care management within the Patient Centered Medical Home (PCMH) integrated model. The candidate will be certified (waivered) in the treatment of drug addiction and will assess and assist with staff development related to behavioral health and addiction treatment.  The primary function of the Psychiatrist is to diagnose, prescribe medication, monitors medications side effects and participates in the treatment process.

  • QUALITY COORDINATOR

    QUALITY COORDINATOR

    The primary function of the Quality Coordinator (QC) is to engage LCH staff in doing the right things right to maximize the efficiency of the time and energy we spend serving our members.  The QC ensures the efficient and effective administrative back office filing and indexing of patient documents, internal and external (insurance) credentialing, closes the loop on transitions of care at specialty offices for patients of the LCH health center (Kennett Square, Oxford, and Jennersville), and supplies relevant data for managers and staff to initiate and evaluate Continuous Quality Improvement (CQI) processes such as Plan-Do-Check-Act cycles.

  • SENIOR ACCOUNTANT

    SENIOR ACCOUNTANT

    The Senior Accountant is responsible for the accuracy, timeliness and efficiency of the day–to–day accounting activities at LCH including month end closing journal entries and duties, balance sheet reconciliations, contract and grant recording and reconciliations, TRNA maintenance and reconciliations, HRSA recording and reconciliations, G/L maintenance, fixed asset maintenance, monthly medical revenue posting to GL & analysis, bi-weekly payroll punch transfers and payroll analysis.
